The YA Summer Camp is set to be an exciting event with water activities such as boat/jet ski activities, swimming, physical activities, and camping. While the Berwick Church of Christ holds General Public & Liability insurance, it is essential to note that this insurance will not extend to cover any personal injury, property damage, or lost/stolen items that may occur during the course of the activities at the YA Summer Camp.

Waiver:

• This is to acknowledge and confirm the Liability Waiver for Berwick Church of Christ and the FORGE YA Summer Camp in relation to my participation in all activities/games associated with the YA Summer Camp.
• I fully understand the terms and conditions outlined in the Liability Waiver, and I willingly agree to abide by them. I acknowledge the risks involved in participating in the activities/games associated with the Summer Camp and release Berwick Church of Christ from any liability concerns associated with property damage, incident, accident, or illness that may arise as a result of my participation.
• I acknowledge that in the event of any incident, accident, or illness at the FORGE YA Summer Camp, the organizers have my consent to secure necessary medical assistance, and I understand and agree that any expenses related to such medical assistance will be covered by either myself or relevant family members.
• I understand that the insurance policies in place for YA Summer Camp specifically cover public liability and professional indemnity for the event itself and its event organizers. However, understand this coverage does not extend to the registered participants and the Church in the case of any personal injury, whether pre-existing or new.
• By participating in the YA Summer Camp, you agree to release, waive and hold harmless the Church, Berwick Curch of Christ, it's elders, staff and volunteers from any claims arising from personal injury sustained during the activities, except in the cases where such claims reesult from a negligent or wilful act or omission of the Church and/or its representatives.
• I confirm that the details provided during the registration submission are true and correct.

Furthermore, you (or the child under your care) waive the right to sue the Church for any issues related to the services, events, general activities, and water activities provided at the YA Summer Camp.

CONSENT

I consent to my child attending the Young Adult Summer Camp 2024.  I give consent for my child to participate in all activities while attending this event.  I acknowledge the staff and volunteers of Berwick Church of Christ will take all possible care but will not be held responsible for any unforeseen accident, illness or loss of property arising.

I consent that in the event of any illness or accident for the employing of medical or hospital assistance as my child requires.  I understand that I shall be notified as soon as practicable should such an incident occur.  I accept the responsibility for payment of any expenses incurred for such assistance.

If during this event my children's behaviour is deemed by Berwick Church of Christ staff or volunteers to be inappropriate or compromises the safety of other children or volunteers, I understand that I will be contacted as soon as possible and my child may be removed from the program or activity as necessary.

I give permission to Berwick Church of Christ to photograph or video my child and to use his/her image, photograph or video in printed or digital materials, on the Church website and social media accounts produced by the Berwick Church of Christ and any of its related ministries.  We DO NOT include the name of your child or "tag" them.  Possible uses include, but are not limited to, reports, brochures, websites, social media posts, promotional videos and multimedia presentations.

I am aware that the Youth Leaders are bound by the Berwick Church of Christ Policies and Code of Conduct and that they are committed to providing a Safe Church environment at all time.
