This Application Form is to be completed by anyone who wishes to serve.

Anyone desiring to be involved at the Church must be known by an Elder, Ministry Leader or member of Staff for at least six (6) months and have been in regular attendance at the Church for at least six (6) months, as detailed in our Volunteer Recruitment Policy.

This Form will be kept indefinitely with the confidential Safe Ministry Records which are maintained by the Safety Contact Person.

Please give names and contact details of 2 people who know you well but are not related to you.

Note: If you are not in attendance at Berwick Church of Christ or if your attendance at Berwick Church of Christ is less than 6 months, then one your referees needs to be a Pastor from the Church in which you are/where attending and has known you for a minimum of 6 months.

All volunteers are required to hold a current Working With Children Check (if aged 18 years and over) nominating the church as an organisation they volunteer with, whether working directly with children or not.

The Cafe Team or Food Pantry is required to comply with Australian food safety laws and standards to ensure he food we provide is safe to eat and free of any contamination.

To ensure everyone understands these food safety laws and standards, we require all Food Pantry voluneers to hold a Food Handlers Certificate.  

If you do not have a Certificate then you can undertake the free online program "DoFoodSafely" provided by the Department of Health Victoria. DoFoodSafely is designed to enable you to understand how to safely work with, and handle food. Click here to access the free online program.
